Review of USCG Paperwork Packet and processing to NMC REC Center.

Please use the forms, checklist and instructional video at:

www.captainschoolkeywest.com/paperwork   to complete all forms and put into a pdf packet.  The packet must be one attachment that is smaller than 20MB.  

You will email the packet to:  paperwork@captain-schools.com  as an attachment not as a link!! 

Please put in the following order before scanning.  Please discard any instructional pages as they are not needed:

Conviction form (if necessary)

Drug Screen (only cg719p!! Please do not send carbons,etc…)

Sea Service Form(s) and proof of ownership (if your boat)

Pay.Gov Receipt

Copy of TWIC card receipt or copy of front and back of actual card  

All diplomas for classes taken with us (please none of the online completion certificates)

CPR/AED/1st Aid Card (if not including diploma from our class)

Physical (10 pages)
